ELEMENTS PHARMACY located at
12602 VENTURA BLVD, STUDIO CITY.
Please bring:
-Proof of age (see examples below)
-Insurance card
-White vaccine record card
Additional Instructions:
-You will be asked to wait 15-30 min following the vaccine for observation for any severe allergic reactions
The following are examples of acceptable documentation for proof of age:
- Passport (foreign country or expired okay)
- Birth certificate
- Medical document/records (including immunization records) from medical provider, clinic, or doctor
- Any official document that includes name and date of birth (for example, school record)
